What Is Airway Orthodontics?

Airway orthodontics is orthodontic care planned around function as well as alignment. Narrow upper jaws, low tongue posture, and crowded arches can all reduce the room available for comfortable nasal breathing. Guiding that development while a child is still growing is far simpler than trying to correct the pattern once growth is finished.

In practice, that means we look at more than the bite during an exam. We look at how a child breathes at rest, how the palate is shaped, how the jaws relate to one another, and whether the habits we can see match the symptoms parents are describing at home.

Signs Your Child May Need an Airway Evaluation

Airway concerns rarely announce themselves. The signs show up in sleep and in daily habits long before they show up in the teeth. Watch for:

  • Mouth breathing or a chronic open-mouth resting posture
  • Snoring, noisy breathing, or restless sleep
  • Chronic congestion or allergies
  • Daytime tiredness or trouble focusing
  • A narrow palate, crowded or overlapping teeth
  • Crossbite, underbite, or overbite
  • Thumb-sucking past age 4
  • Speech or articulation concerns
  • Difficulty chewing, or complaints when eating

None of these confirm an airway problem on their own. Together, they are a good reason to have someone look.

Why Age 7 Is the Right Time to Look

The American Association of Orthodontists recommends a first orthodontic evaluation by age 7. For airway and structural concerns that timing matters even more than it does for alignment: the first adult molars are in, jaw growth patterns are readable, and there is still growth left to guide.

That is also why our early orthodontic treatment and airway evaluations overlap so heavily. Both are about intercepting a developing pattern instead of correcting a finished one. Many children we see at 7 need nothing but monitoring — and knowing that is worth the visit by itself.

What Happens at an Airway-Focused Exam

The visit is a conversation before it is an examination. We ask what you are hearing at night, how your child breathes when they are relaxed and not thinking about it, whether mornings are hard, and what their dentist or pediatrician has already told you.

From there the clinical exam covers arch width and palate shape, how the upper and lower jaws relate, crowding and eruption pattern, resting lip and tongue posture, and the records we need to track growth over time. You leave with a plain-language explanation of what we found, whether it warrants treatment now, and what we would watch for if it does not.

How We Approach Airway-Focused Care

There is no single airway appliance. What we recommend depends entirely on what the exam shows and how much growth is available:

  • Growth guidance and monitoring, with no appliance, when a child is developing normally
  • Palatal expansion to widen a narrow upper arch and create room
  • Space management so permanent teeth erupt where they belong
  • Braces or Invisalign when comprehensive alignment is the right next step
  • Referral and coordination with your pediatrician, ENT, or sleep physician when the findings point beyond orthodontics

That last point is deliberate. Orthodontists do not diagnose sleep apnea. When something we see suggests a medical evaluation is needed, we say so and help you get to the right specialist rather than treating around it.

Is airway orthodontics only for children?
Early treatment is where orthodontics has the most influence, because we can guide growth. Adults with narrow arches and crowding can still be treated, but the options and the goals are different, and we will walk through them at your consultation.

Can orthodontic treatment cure sleep apnea?
No. Sleep apnea is a medical diagnosis made by a physician. Orthodontic treatment can improve jaw structure and nasal airflow, and it is often part of a broader plan, but it is not a cure and we will not present it as one.

At what age should my child be evaluated for airway concerns?
By age 7, in line with the AAO recommendation. If you are seeing snoring or mouth breathing earlier than that, come in earlier — an exam costs you nothing.

Does my Riverside child need a referral to be seen?
