An orthodontist is a dental specialist who completes two to three years of additional residency training after dental school, focused entirely on tooth movement, bite correction, and jaw growth. A general dentist treats overall oral health. Both can be excellent providers, but only an orthodontist is a specialist in straightening teeth.

If you have been told your teeth could be straightened at your regular dental office, you are not imagining a difference in training. It is real, it is significant, and it is worth understanding before you commit to a year or more of treatment. Here is how the two roles compare, and how to decide which one belongs in your care plan.

What Is the Difference Between an Orthodontist and a Dentist?

Every orthodontist starts as a dentist. The path splits after dental school, when a small number of graduates are accepted into an accredited orthodontic residency. That residency is where a provider learns to plan tooth movement in three dimensions, read growth patterns in a developing jaw, and predict how a bite will settle years after the appliances come off.

Can a General Dentist Straighten Your Teeth?

In most states, yes. Nothing in a dental license prevents a general dentist from offering braces or clear aligners, and many dentists do so responsibly for straightforward cases. The question is not whether it is allowed. The question is whether your particular case is straightforward.

Mild crowding in an adult with a stable bite is a very different problem from a deep overbite in a growing twelve-year-old, an impacted canine, or an open bite driven by jaw position. The second group needs a provider who plans those cases every single day.

When a General Dentist May Be a Reasonable Choice

  • Minor crowding or spacing with no bite problem
  • Minor relapse after previous orthodontic treatment
  • Short cosmetic cases limited to the front teeth

When You Should See an Orthodontist

  • Overbite, underbite, crossbite, or open bite
  • Any child under 12 who has not had an orthodontic evaluation
  • Impacted, blocked, or missing teeth
  • Jaw pain, clicking, or a jaw that shifts when you close
  • A case where a previous provider is unsure of the plan

Why Specialist Training Changes the Result

Straightening teeth is not just about making the front six look better in photos. The teeth you see are attached to a bite that has to function for decades. A plan that closes a gap but leaves the back teeth meeting incorrectly can create uneven wear, chewing problems, and relapse.

Residency training is where an orthodontist learns to sequence that work: which teeth move first, how much force is safe, when to use elastics or expanders, and how to finish a case so it holds. That planning ability is the difference you are paying for, whether you choose metal braces or clear aligners.

Do You Need a Referral to See an Orthodontist?

No. Orthodontics is a direct-access specialty, which means you can call and book an evaluation yourself. Many patients come to us on a dentist’s referral, and many others simply call because a child’s teeth are crowding in or an adult finally decided to fix something that has bothered them for years. Both routes are equally welcome.

How to Choose Between the Two

Ask any provider three direct questions before you agree to treatment:

  1. What is your training in orthodontics? A specialist will name their residency program. A general dentist should tell you how many cases like yours they have finished.
  2. What is my diagnosis, not just my treatment? You should hear a description of your bite, not only a promise about your smile.
  3. What happens if my case does not respond as planned? A good answer includes a specific fallback, including referral to a specialist.

If the answers are vague, keep looking. A second opinion costs you an afternoon; a poorly planned case costs you years.

Cost: Does Seeing a Specialist Change the Price?

Most patients assume specialist care carries a premium. In the Riverside and Corona market, the fee ranges overlap heavily. Comprehensive orthodontic treatment generally runs about $3,000 to $8,000 depending on complexity and appliance type, whether the provider is a general dentist or an orthodontist.

Where the numbers actually diverge is in what the fee covers. Ask any provider to confirm in writing whether the quoted fee includes:

  • Diagnostic records: X-rays, photos, and a 3D scan
  • Every adjustment or aligner-change visit for the full treatment period
  • Refinement trays or additional wires if the case needs them
  • Retainers at the end, and replacement policy if one is lost
  • Emergency visits for a broken bracket or a poking wire

A fee that looks $800 lower but excludes retainers and refinements is not lower. This is the single most common reason two quotes for the same mouth look nothing alike.

What Kind of Case Does Each Provider See Most?

Volume matters in a procedural specialty. An orthodontist places brackets, sequences wires, and reads growth records every working day. A general dentist doing a handful of aligner cases per year is not doing anything wrong, but they are operating far from the top of their learning curve on that specific skill.

A direct, fair question to ask any provider: “How many cases like mine did you finish last year, and can I see before-and-after photos of two of them?” Providers who do this work routinely answer that instantly.

Red Flags in Either Setting

  • A treatment plan quoted before records are taken. No one can plan tooth movement without X-rays.
  • A discount that expires today. Orthodontics is a two-year relationship, not a flash sale.
  • No discussion of your bite. If the whole conversation is about the front six teeth, the back teeth are being ignored.
  • No retention plan. If retainers are an afterthought, relapse is being priced into your future, not theirs.
  • Reluctance to refer. Any provider unwilling to name the point at which they would send you to a specialist is telling you something important.

Green Flags Worth Looking For

  • A written diagnosis in plain language, not just a treatment name
  • Records reviewed with you on screen
  • A stated treatment duration with a range, and what would change it
  • Coordination with your general dentist for cleanings and any restorative work
  • Clear elastic and retainer instructions given in writing

A Note for Families in Riverside and Corona

Two situations come up constantly in our area and both belong with a specialist. The first is the seven- to nine-year-old whose permanent teeth are erupting into a crowded arch; early evaluation determines whether growth can be used to your advantage or whether waiting is smarter. The second is the adult who had braces as a teenager, stopped wearing retainers in college, and now has front teeth that have drifted back. That second group is usually a shorter, more affordable case than they expect, and delaying rarely makes it cheaper.

Is an orthodontist more expensive than a dentist for braces?

Not necessarily. Fees in the Riverside and Corona area overlap heavily, and specialist practices often treat enough volume to price competitively. Compare total treatment fees, including retainers and follow-up visits, rather than monthly payments alone.

Can my dentist and orthodontist work together?

Yes, and they should. Your dentist continues your cleanings and restorative care during orthodontic treatment, while your orthodontist manages tooth movement. Good providers share records and coordinate timing.

What does board certified mean for an orthodontist?

Board certification is a voluntary step beyond residency in which an orthodontist submits finished cases for peer review by the American Board of Orthodontics. It is a signal that a provider is willing to have their results examined by other specialists.

At what age should a child first see an orthodontist?

The American Association of Orthodontists recommends a first evaluation by age 7, when the mix of baby and permanent teeth reveals developing bite problems. Most children do not start treatment that early, but the evaluation sets the timeline.

Do I still need my regular dentist during orthodontic treatment?

Absolutely. Cleanings become more important with braces, not less, because brackets and wires create more places for plaque to collect.
